Physical Therapist (Nursing)

Duty of the Position

The Licensed Physical Therapist coordinates and provides Physical Therapy Services to patients in their homes. Supervises care delivered by Licensed Physical Therapist Assistants and Home Health Aides (as appropriate).

Position responsibilities

Provides individualized skilled care as indicated
1. Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
a. Provides individualized skilled care as indicated.

Assesses patients in their homes
2. Assesses patients in their homes to determine level of physical function.
a. Uses independent professional judgement to determine standardized evaluative methods and non-standardized measures which will best meet the needs of the patient and yield the best assessment of the physical dysfunction(s).
b. Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
c. Determines the nature of the physical dysfunction, the severity of the dysfunction, the prognosis for improvement, and recommendations for treatment.
d. Documents a comprehensive evaluation report and informs the physician, patient/family, and other team members of the evaluation results and recommendations.

Establishes and implements a physical therapy treatment
3. Establishes and implements a physical therapy treatment plan in collaboration with the patient's primary physician and other team members.
a. Develops plan of care including treatment and outcome measures.
b. Provides individualized treatment as indicated.

Observes, records and reports to the physician
4. Observes, records and reports to the physician any reaction to treatment or changes in the patient's condition.
a. Evaluates, documents progress and revised patient's plan of care as needed a minimum of every 62 days in collaboration with the physician.

Instructs the family on the physical therapy program
5. Instructs the family in the patient's total physical therapy program.
a. Counsels the patient and family regarding the physical dysfunction and provides strategies for optimal improvement.

Participates actively in patient care conferences
6. Instructs family members, in-home aides and other health team personnel in performing appropriate therapy treatment.
a. Participates actively in patient care conferences.

Performs supervisory visits of PTA's
7. Performs supervisory visits of the Licensed Physical Therapist Assistants a minimum of every 30 days.

Demonstrates professionalism
8. Demonstrates professionalism.
a. Maintains current knowledge and skill base necessary to the provision of quality home physical therapy.
b. Adheres to the Agency's policies and procedures.
c. Attends regularly scheduled staff meetings and in-service classes.
d. Uses tact and courtesy in all interactions with the staff, patients, families, and referral sources.
e. Promotes a positive image of the Agency and supports Administration in its goals and objectives.
f. Handles inquiries and complaints discreetly and effectively per policy and procedure.

Assist in education
9. Assists in the education of medical students/residents, physical therapy students and physical therapist assistant students.
a. Serves as a preceptor in the clinical training of medical students/residents, physical therapy students and physical therapist assistant students for home health.
b. Assists with the orientation of new staff and others as assigned.

Job Conditions

The weight of a raise patient (assist with transfer) with gait belt is 179.3 pounds. This is a bilateral lift. This object is lifted between Knuckle height and Elbow/Chest height. NOTE: This should be performed as a team lift. Would be required if patient transfer was not completed properly and patient ended up on floor. The weight of a patient oxygen tank is 11.7 pounds. The patient oxygen tank is carried a distance up to 1000 feet from unit floor to rehab room. This is a unilateral or bilateral carry.
The push force required to overcome inertia to move a room recliner is 17.5 pounds. This push is generally completed at a height of 43.5 inches above the ground. This push is completed with bilateral upper extremities, with the forearms rotated into pronation and the hands utilizing a hook grip pattern. Hand spread when completing this push is 29 inches. NOTE: Back of recliner head (handle) at 43.5 inches (when upright) arms at 28.5 inches above ground and 29 spread.
The push force required to overcome inertia to move a patient on faaborgliften lift is 23.6 pounds. This push is generally completed at Elbow/Chest height. This push is completed with bilateral upper extremities, with the forearms rotated into pronation and the hands utilizing a cylindrical grip pattern. Hand spread when completing this push is based on size of patient. NOTE: Measured 235#, 72" tall male.
The push force required to overcome inertia to move a wheelchair (manual) up ramp is 46.7 pounds. This push is generally completed at a height of 33.0 inches above the ground. This push is completed with bilateral upper extremities, with the forearms rotated into pronation and the hands utilizing a cylindrical grip pattern. Hand spread when completing this push is 20 inches.

Qualifications

Basic Life Support (BLS) Certification
Minimum of Bachelor of Science Degree in Physical Therapy required.
Minimum of two years of clinical experience as a Physical Therapist preferred.
Licensure by the North Carolina Board of Physical Therapy required.
Must have a valid North Carolina driver's license with minimum North Carolina automobile insurance requirements.
Must maintain adequate /dependable transportation in order to perform specific job functions.
