Duty of the Position

The ADON/QA Manager position will be required to work alongside the DON to case manage patients throughout the complete life cycle of care, from Admit to Discharge.

Position responsibilities

• The ADON/QA Manager position will be required to work alongside the DON to case manage patients throughout the complete life cycle of care, from Admit to Discharge.
• Tasks include oversight of QA team, QAPI management, full chart review, agency compliance measures, in-services & training.
• The candidate will also review utilization, productivity & improving HHRG and outcome scores.
• Ensures that new skill training and ongoing competency verification is completed annually.
• Audits, completes, and monitors or provides follow up for all incident and occurrence reports.
• Provides direct patient care as needed in state regulations for RN
• Acts as a resource for the Executive Leadership Team for operational and clinical decision-making.
• Participates in on-call rotation.
• Visits all patients within their assigned region at least annually. Patients identified as having >16 hr/day of aide care >12 hours of nursing per day are to be visited quarterly.
• Other duties as assigned.

Job Conditions

State licensed as a Registered Nurse or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NCL),
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.

Qualifications

• State licensed as a Registered Nurse or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NCL),
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• 2+ years nursing experience in the home health industry
• Experience in OASIS charting and review, Home health coding
• Excellent interpersonal skills and ability to communicate effectively. Compassionate caring personality with Demonstrated proven decision-making skills. Able to read, write, and comprehend English. If required to make visits, need proof of Hepatitis profile
