Duty of the Position

-Complete Past medical and Present medical condition during admission.
-Assess and chart observations of the patient's condition at each visit
-Complete evaluation tasks by reviewing medication and vital signs
-Administer Physician-prescribed medication, Including IV fluids, IV antibiotics, Blood Transfusion, and other infusion therapist medication as order by physician
-Wounds care with dressing change assessment of the healing progress
-Educating patients and families on home health care procedures and strategies
-Coordinate with physicians, physical therapists, occupational therapists, Speech therapist and the rest of the care team.
-Keep the patient comfortable through palliative care. --
-Diabetes education, COPD education, HTN education, CHF, Obesity education, Weight manage with patient and family.
-Recommend tools or devices that may improve the quality of life for the patient.
